By providing this information you are agreeing for "My Photographer" (Alicia Hester-Eatmon) and any additional assistants or photographers necessary, to photograph your child for the event listed above. The images taken by those affiliated with My Photographer may or may not be used in marketing, portfolio, or any other way necessary. My Photographer does reserve the right to do with the pictures as they please but is, herself, a mom and will NOT sell your child's images to a third party for any reason, without your permission and compensation. And finally you also agree to the following: 1. All recordings, photographs, videos, are not to be taken without permission of the photographer. B) Patience during possible wait time. C) Have fun!---I promise to be patient, spend the same quality of time with each child (even though some require more/less direction), get the best shots, and fulfill any service you pay for! I love to eat, veggies and fruit will never be turned down! Upon receiving this form, you will be sent a PayPal notification. Upon receiving your payment of $15 per child you will be given your numeric position for the posed shots. If you wish to purchase any other products, let me know. If I don't have them listed, let me know and I will find them.

$15- You'll receive a 10x10 Memory Mate which will have 3-4 different images and a cd with a print release of your child's individual shots and their action shots captured during the game. The team photo will not be on the cd.
